How can I request an vaccination against COVID-19 exemption to travel out of Canada?

All passengers who are 12 years and 4 months of age or older departing from Canada must present a COVID-19 vaccination certificate showing they have been fully vaccinated at least 14 days prior to flight departure.

Very limited exemptions to vaccination will be allowed of which the following will be administered by the airlines:

Copa Airlines will accept/approve or refuse temporary passenger exemptions on detailed guidance from the authority.

If the exemption is approved,

  • Passengers are required to carry the completed exemption form and proof of the operator’s acceptance/approval with them during their travel journey.
  • During the check-in, must present a negative COVID-19 test performed no more than 72 hours prior to departure of flight to Canada.

How to request a temporary exemption?

Temporary exemption for passengers due to medical inability to be vaccinated

    1. Submit the Temporary COVID-19 Vaccination Exemption Request Form minimum 21 days in advance and,
    2. Attach the Temporary exemption due to medical inability form with one of the following:
      • Confirmation and signature from a Canadian medical doctor / nurse practitioner on the form; or,
      • A medical letter or credential issued by a Canadian Province/Territory indicating a medical inability to be vaccinated annexed to the form.
    3. The form must include:
      • Contact details (civic address/phone number) for the medical doctor or nurse practitioner, as well as their license number.
      • Sign and date of by the medical doctor or nurse practitioner
      • Sign and date of the requestor.

The temporary exemption form for passengers due to medical inability to be vaccinated is valid for 1 year following the date of signature by the medical doctor or nurse practitioner.

 

Temporary Exemption for Essential Medical Services or Treatment

  1. Submit the Temporary COVID-19 Vaccination Exemption Request Form minimum 14 days in advance and,
  2. Attach the Temporary exemption form due to essential medical services or treatment with:
    • The confirmation from a Canadian medical doctor or nurse practitioner that contains the date, name, and civic address of the facility where the essential medical services or treatment appointment will be.
    • A brief description by the Medical Doctor/Nurse Practitioner.
    • In all cases, a completed exemption form must be submitted along with the supporting documentation.
  3. The form must include:
    • Contact details (civic address/phone number) for the medical doctor or nurse practitioner, as well as their license number.
    • Sign and date of by the medical doctor or nurse practitioner
    • Sign and date of the requestor.

The temporary exemption form for essential medical services or treatment is valid only for one trip. Further travel will require the submission of a new form.

 

Temporary Exemption for Passengers due to Sincere Religious Belief

  1. Submit the Temporary COVID-19 Vaccination Exemption Request Form minimum 21 days in advance and,
  2. Attach the Temporary exemption form due sincere religious belief that includes a sworn, solemn oath of conviction co-signed by a Commissioner of Oaths.
  3. The form must be:
    • Signed and dated by a Commissioner of Oaths or may be certified by the Commissioner of Oath’s affixing a stamp/seal and date.
    • Signed and dated by the requestor.

The temporary exemption form for passengers due to sincere religious belief is valid only for one trip. Further travel will require the submission of a new form.
